*This question is specific to OUA students only*

Please refer to Section C of the form for examples of supporting documents. When the supporting documentation is a medical certificate it must:

  • be a signed statement from a registered medical practitioner, health practitioner or approved health specialist.*
  • specify the period of time during which the medical condition has, or will, affect a student's participation and/or attendance.

Medical certificates are legal documents and must contain the practitioner's stamp. Please request your medical practitioner to stamp the document.

Certificates must be dated on the day on which they were written. When a medical certificate certifies a period for which a student is/was unable to participate in or attend, which is prior to the date of the medical consultation, the medical certificate should include supplementary remarks to explain any discrepancy.

When submitting documents electronically please refer to the guidelines.

*Refer to the General Definitions at the end of the Assessment Policies and Procedures Manual for the full definition of medical certificate and who can issue a medical certificates.
